Job summary

Optimal Care in Carmel, IN is seeking a Physician Services leader to deliver advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings, with autonomy to focus on thoughtful, patient-centered care that improves comfort and quality of life.

You will supervise Nurse Practitioners and Physician Assistants, coordinate care with families and interdisciplinary teams, and mentor clinicians to elevate outcomes while respecting the patient-provider

Qualifications

• MD or DO with unrestricted state medical license.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Prior supervisor/management experience required.
• Reliable transportation, valid driver's license and auto insurance.

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in homes or care settings.
• Lead and mentor Nurse Practitioners and Physician Assistants, ensuring coordinated, high‑quality care.
• Coordinate care with patients, families and interdisciplinary teams to improve outcomes.
• Help reduce hospitalizations and enhance quality of life for patients and families.
