Job summary

HarmonyCares is seeking a Value-based Physician Leader to provide clinical oversight for a growing primary care team in Kalamazoo. This on-site role oversees APPs delivering chronic condition management to high-risk geriatric patients, with a Monday through Friday schedule and no weekends or evenings.

The clinician will lead a pod in a physician-driven, team-based model, collaborating with regional leadership and ensuring high-quality, cost-effective care.

Qualifications

• Must be a Doctor of Medicine or Osteopathy.
• Bachelor’s Degree required.
• Valid driver’s license with clean driving record.
• Active CPR certification required.

Responsibilities

• Act as on-site supervising Physician for assigned APPs and collaborating physicians; ensure compliance with state regulations.
• Lead clinical pod performance, including diagnosis, documentation, patient safety and quality metrics.
• Develop APP clinical development and foster high-quality, evidence-based care.
• Lead high-risk patient huddles, ensure care plans involve the full care team.
• Collaborate with regional leadership to improve care delivery and EMR efficiency.
