Job summary

Optimal Care in Maumee, OH is seeking a Physician Services clinician to provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to deliver thoughtful, patient-centered care that improves comfort and quality of life.

You’ll conduct comprehensive evaluations, manage acute and chronic conditions, and coordinate care with families and interdisciplinary teams while mentoring Nurse Practitioners and Physician

Qualifications

• Unrestricted State medical license required.
• Board certification in family practice, internal medicine, geriatric or palliative care.
• Experience in supervision/management preferred.
• Reliable transportation, valid driver’s license, and valid automobile insurance.

Responsibilities

• Conduct comprehensive medical evaluations and manage acute and chronic conditions.
• Provide leadership and oversight for Nurse Practitioners and Physician Assistants.
• Coordinate care with patients, families, and interdisciplinary teams.
• Improve patient outcomes and reduce hospitalizations.
