Job summary

Optimal Care is seeking a Physician to deliver advanced primary and specialty care, often in patients' homes or care settings, with autonomy to focus on patient-centered care. You will lead clinical teams, coordinate care, and improve outcomes for geriatric and medically complex patients.

You will mentor Nurse Practitioners and Physician Assistants, ensure high-quality, coordinated care, and collaborate across disciplines to enhance quality of life.

Qualifications

• MD/DO degree and active state license to practice medicine.
• Board certification in family practice, internal medicine, geriatric, or palliative care.
• Prior supervisor or management experience is required.

Responsibilities

• Perform comprehensive medical evaluations and manage acute and chronic conditions.
• Develop individualized treatment plans and coordinate care with patients, families, and interdisciplinary teams.
• Provide medical leadership and mentee supervision for NPs and PAs; oversee quality and outcomes.
