Job summary

Optimal Care is seeking a Physician in Physician Services to deliver advanced primary and specialty care, frequently in patients' homes or care settings, with autonomy to focus on patient-centered care that improves comfort and quality of life.

You will conduct evaluations, manage conditions, and coordinate with families and teams while mentoring NPs and PAs, contributing to improved outcomes and reduced hospitalizations in the Jackson, MI service area.

Qualifications

• MD or DO with state license to practice medicine
• Board certification in family practice, internal medicine, geriatric or palliative care
• Prior supervisor/management experience
• Reliable transportation with valid driver's license and auto insurance

Responsibilities

• Provide advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings.
• Conduct comprehensive medical evaluations and manage acute/chronic conditions; develop individualized treatment plans.
• Coordinate care with patients, families, and interdisciplinary healthcare teams; mentor clinicians.
• Provide medical leadership and oversight for Nurse Practitioners and Physician Assistants; ensure high-quality, coordinated care.
