Job summary

Qualifications

• Must reside in Illinois Fayette County or surrounding areas.
• Willing and able to travel up to 75% to meet with members.
• Reliable transportation with mileage reimbursement per policy.
• Minimum 2 years of experience in Behavioral Health, Social Services or Human Services.
• Minimum of two years of case management experience.
• Valid Driver's license, preferably out of IL.

Responsibilities

• Conducts comprehensive evaluation of referred members' needs/eligibility and recommends an approach to case resolution by evaluating benefit plans and available programs.
• Identifies high risk factors and service needs and coordinates referrals to clinical case management or crisis intervention.
• Coordinates and implements assigned care plan activities and monitors progress.
