Responsibilities

• Facilitate appropriate healthcare outcomes for waiver/LTSS members by providing care coordination, support and education.
• Evaluate members’ needs and eligibility and coordinate care plan activities and progress.
• Enhance medical appropriateness and quality of care by consulting with case managers, supervisors and physicians to overcome barriers.
• Monitor, evaluate and document care in compliance with guidelines and policies.
