Qualifications

• Minimum of two (2) years of case management experience.
• Microsoft Office and electronic health record experience.
• Must possess reliable transportation and be willing and able to travel up to 75% of the time. Mileage is reimbursed by our company expense reimbursement policy.
• Valid Illinois Driver's license and must reside in one of the above zip codes or surrounding areas.

Responsibilities

• Provide care coordination, support and education for waiver/LTSS members.
• Conduct in-person home visits with members.
• Travel 50-75% of the time to meet with members face to face.
• Manage a full caseload of waiver members.
