Job summary

CVS Health is seeking a Field-based LTSS Case Management Analyst to support waiver members in Illinois. You will conduct care coordination, education, and evaluation to achieve optimal outcomes while traveling 50-75% to meet with members face to face.

Responsibilities include reviewing member needs, coordinating care plans, and collaborating with case managers and physicians. A Bachelor’s or Master’s in Behavioral Health or Human Services is required, with at least 2 years in related fields.

Qualifications

• Must reside in Illinois in Fayette County or surrounding areas.
• Willing and able to travel up to 75% to meet with members face to face.
• Reliable transportation; mileage reimbursed per policy.
• Minimum 2 years of experience in Behavioral Health, Social Services, or Human Services.
• Minimum of two years of case management experience.
• Valid driver’s license; IL residency preferred.

Responsibilities

• Conduct comprehensive evaluation of referred members’ needs/eligibility and advise on approach to case resolution.
• Coordinate and implement care plan activities; monitor progress.
• Identify high risk factors and coordinate referrals to clinical case management or crisis intervention.
• Consult with case managers, Supervisors, Medical Directors to overcome barriers and achieve goals.
• Promote member engagement and healthy lifestyle choices through coaching and support.
