Job summary

CVS Health is seeking an Analyst, Case Management Field to support LTSS members in Illinois. This field-based role requires 50–75% travel, in-person visits, and managing a full caseload to ensure waiver/LTSS goals are met.

Responsibilities include evaluating member needs, coordinating care plans, and collaborating with clinical teams to optimize outcomes. A bachelor’s or master’s degree in Behavioral Health or related field is required.

Qualifications

• Requires residence in Illinois, Fayette County or surrounding areas.
• Minimum 2 years of experience in Behavioral Health, Social Services, or Human Services.
• Minimum 2 years of case management experience.
• Valid driver's license, preferably from IL.

Responsibilities

• Evaluate referred members’ needs and eligibility and coordinate care planning.
• Identify high-risk factors and coordinate referrals to clinical case management or crisis services.
• Coordinate and monitor care plan activities and progress.
• Coordinate with supervisors, Medical Directors, and health programs to improve outcomes.
• Utilize care management tools to support waiver/LTSS members.
