Qualifications

• Must reside in Illinois west side Chicago area.
• Valid Illinois driver's license.
• Willing to travel up to 75% with mileage reimbursed.
• Minimum of two years of case management experience.

Responsibilities

• Coordinate and manage care plans and member visits.
• Evaluate member needs and eligibility and coordinate services.
• Collaborate with clinical staff to ensure outcomes and quality.
• Provide education and support to empower members in decision-making.
