Job summary

CVS Health in Illinois is hiring a Case Management Field Analyst to support LTSS members with care coordination, education, and program implementation. This full-time role requires up to 75% travel and in-person visits, with a full caseload and quarterly member visits.

You will evaluate member needs, coordinate care plans, and collaborate with clinical teams to ensure compliant, quality outcomes. A Bachelor's or Master's in Behavioral Health or related field is required, plus 2 years of case

Qualifications

Minimum 2 years of experience in Behavioral Health, Social Services or Human Services field.
Minimum of two (2) years of case management experience
Must reside in Illinois Fayette County or surrounding areas
Willing and able to travel up to 75% of their time to meet with members face to face
Valid Driver's license, preferably out of IL

Responsibilities

Evaluate referred members' needs and eligibility and recommend an approach to case resolution and/or meeting needs by evaluating benefit plans and available internal or external programs/services.
Identify high risk factors and service needs that may impact member outcomes and care planning; refer as appropriate.
Coordinate and implement assigned care plan activities and monitor progress.
