Responsibilities

• Provide care coordination, support and education to LTSS members.
• Evaluate member needs/eligibility and tailor care plans.
• Identify high risk factors and coordinate care plan activities.
• Escalate quality of care issues and coordinate with clinical teams.
• Coach members to engage with providers and sustain healthy choices.
