Qualifications

• Minimum of two (2) years of case management experience.

• Valid Illinois Driver’s license and reliable transportation.

Responsibilities

• Evaluate members' needs/eligibility and plan care resolutions.
• Coordinate and monitor assigned care plan activities.
• Consult with case managers and supervisors to improve outcomes.
