Qualifications

• Requires at least two years of case management experience.

Responsibilities

• Evaluate referred members' needs and eligibility and plan case resolution.
• Coordinate and monitor care plan activities and progress.
• Identify high-risk factors and escalate to appropriate care management when needed.
