Qualifications

• Minimum of two years of case management experience.
• Must reside in Illinois Macoupin County & Surrounding Areas, IL.
• Must possess reliable transportation and be willing to travel up to 75%.
• Valid Illinois Driver’s license.
• Experience with Microsoft Office and EHR systems.

Responsibilities

• Evaluate referred members’ needs and eligibility and plan case resolutions.
• Coordinate care plan activities and monitor progress.
• Facilitate collaboration with clinical teams to overcome barriers.
• Provide coaching and education to empower members in decisions.
• Ensure documentation aligns with regulatory guidelines.
