Job summary

CVS Health is seeking a Case Management Field Analyst to support the LTSS program in Illinois, traveling 50-75% and conducting in-person visits with waiver members to coordinate care plans and outcomes.

You will evaluate member needs, coordinate care with clinicians, and document progress while complying with regulatory requirements. A Bachelor's in Behavioral Health or related field is preferred, with at least 2 years of case management experience.

Qualifications

• Must reside in Illinois (Fayette County or surrounding areas) and be willing to travel up to 75%
• Valid Driver's license preferred, mileage reimbursement eligibility
• Minimum 2 years of experience in Behavioral Health, Social/Human Services
• Minimum 2 years of case management experience

Responsibilities

• Conducts evaluation of referred members and determines approach to case resolution
• Identify high-risk factors and coordinate referrals to clinical management or crisis interventions
• Coordinate and implement care plan activities and monitor progress
• Utilize care management tools to ensure appropriate health outcomes and member engagement
• Maintain documentation in compliance with regulatory and accreditation guidelines
