Job summary

CVS Health Corporation is seeking an Analyst, Case Management Field for the LTSS team based in Rock Island County, Illinois. The role requires travel 50-75% and in-person quarterly member visits to coordinate care and manage waiver members.

You’ll evaluate needs, coordinate care plans and engage with clinical teams to ensure optimal outcomes and adherence to regulatory guidelines. A bachelor’s degree in human services and two years of case management are expected.

Qualifications

• Must reside in Illinois in Rock Island area.
• Valid Illinois Driver’s license.
• Willing and able to travel up to 75% of time to meet with members face to face.
• Reliable Transportation; mileage reimbursement per policy.
• Minimum of two years of case management experience.
• Microsoft Office and electronic health record experience.
• If managing HIV/AIDS HCBS Waiver members, experience with racial/ethnic minorities and at least one of: domestic abuse; LBGTQ+ community; Persons living with HIV/AIDS; Persons with substance use disorders.

Responsibilities

• Provide care coordination, support and education to waiver/LTSS members.
• Evaluate member needs/eligibility and plan case resolution.
• Coordinate and monitor care plan activities and progress.
• Collaborate with case managers, supervisors, Medical Directors and health programs.
• Escalate quality of care issues and use negotiation to secure appropriate services.
• Coach members to engage with providers and adopt healthy behaviors.
