Job summary

VitalCaring is seeking a Care Transition Coordinator – Hospice to connect patients, families, and healthcare partners with its hospice services. This field-based role serves as the primary liaison between referral sources and clinical teams, ensuring timely transitions of care and strong partnership development.

You will drive sales activities, build relationships with hospitals and physicians, and coordinate care transitions to improve patient outcomes while supporting organizational growth.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years of healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain strong relationships with referral partners through consistent communication and service excellence.
• Develop and execute strategies to grow market share and drive referral volume.
• Track, manage, and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
