Job summary

VitalCaring Group in the United States is seeking a Care Transition Coordinator to connect patients, families, and referral partners with hospice services. This field-based role focuses on timely transitions and growing referral volume within the assigned MS territory.

You will build relationships with hospitals, physicians, and case managers, manage CRM documentation, and collaborate with clinical teams to ensure smooth admissions and high-quality patient care.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field or equivalent experience.
• Minimum 2 years in healthcare or related sales.
• Strong communication, relationship-building and influencing skills.
• Proficiency with Microsoft Office and CRM systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as liaison between referral sources, patients, caregivers and clinical teams to ensure seamless care transitions.
• Execute structured sales calls including planning, relationship building, needs discovery and closing.
• Maintain strong relationships with referral partners through consistent communication and service.
• Develop strategies to grow market share and drive referral volume.
• Track, manage and document sales activity across referral sources using CRM tools.
• Achieve established referral and admission goals.
