Job summary

VitalCaring Group is seeking a Care Transition Coordinator to connect patients, families, and partners with hospice services. This field-based role focuses on referrals, care coordination, and partnership development to drive patient access and organizational growth.

The candidate will partner with hospitals, SNFs, and physician offices, manage CRM documentation, and work with clinical teams to ensure timely admissions. Travel within assigned territory is required.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience)
• Minimum of two (2) years of healthcare or related sales experience
• Proficiency with Microsoft Office and CRM systems
• Demonstrated ability to work independently and drive results

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions
• Execute structured, effective sales calls including pre-call planning, relationship building, needs discovery, and closing
• Build and maintain strong relationships with referral partners through consistent communication and service excellence
• Develop and execute strategies to grow market share and drive referral volume
• Track, manage, and document sales activity across referral sources using CRM tools
• Achieve established referral and admission goals
