Job summary

Gentiva Hospice is seeking a compassionate RN Case Manager to lead patient care in home settings, coordinating with physicians, LPNs, CNAs, social workers, chaplains, and other team members to build personalized plans of care and ensure comfort, dignity, and family education.

You will perform comprehensive assessments, document timely care plans, provide symptom management, on‑call coverage when required, and communicate changes to the team while promoting regulatory compliance and excellence in

Qualifications

• Current license to practice nursing in the state of employment.
• Minimum of 1 year RN experience in hospice, palliative care, or home health.
• CPR certification.
• Current automobile insurance and valid driver's license.
• Ability to work in patients' homes or other clinical settings.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telecommunication.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams with physicians and other staff.
• Educate patients and caregivers about disease process, self‑care, medications, nutrition, and end‑of‑life care.
• Communicate changes in condition to the care team for timely interventions.
• Document clinical records accurately and maintain infection control and regulatory standards.
• Support on‑call and patient support activities as applicable.
