Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home settings. You will perform initial assessments, develop plans of care, and coordinate with physicians, social workers, chaplains, and other team members to deliver personalized end-of-life care.

You will educate families, manage symptoms, document care plans, and ensure regulatory compliance while supporting patients and loved ones with dignity, comfort, and empathy.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and families on disease, end-of-life care, medications, and self-care.
