Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home or in related settings. You will perform comprehensive assessments, develop plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains.

You will support patients and families with education, symptom management, and bereavement guidance while ensuring accurate documentation and regulatory compliance. ARegistered Nurse license and hospice experience are preferred.

Qualifications

• Active Registered Nurse license in the state of employment.
• Minimum 1 year RN experience in acute care, hospice, home health, or similar.
• Hospice or palliative care experience is preferred.

Responsibilities

• Be the primary contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care in patients' homes or equivalent settings.
• Perform initial and ongoing assessments to monitor condition and adjust care.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on safe patient care and provide reassurance.
• Communicate patient condition updates to the care team for timely interventions.
• Maintain up-to-date clinical documentation and follow infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate, efficient care.
