Job summary

Gentiva Hospice seeks an experienced Hospice RN Case Manager to lead patient care in home and community settings. You will perform comprehensive assessments, create personalized plans of care, and collaborate with physicians, social workers, and other team members to ensure compassionate, evidence-based end-of-life care.

The role emphasizes education for patients and families, symptom management, and timely documentation in compliance with regulatory standards.

Qualifications

• Active RN license in the state of employment.
• 1+ year RN experience in hospice, home health, or acute care preferred.
• CPR certification is required or current.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver and document skilled nursing interventions aligned with goals of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
