Job summary

Gentiva Hospice is seeking a committed RN Case Manager to deliver high-quality, in-home hospice nursing. You will perform comprehensive assessments, implement care plans, and coordinate with physicians, CNAs, social workers, and chaplains to ensure compassionate, patient-centered care.

Responsibilities include symptom management, education for patients and families, on-call duties as assigned, and accurate documentation.

Qualifications

• Current license to practice nursing in the state of employment
• CPR Certification
• Current automobile insurance and valid driver’s license
• Hospice or palliative care experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments and education for patients, families, and caregivers
• Deliver skilled nursing interventions and symptom management aligned with goals of care
• Develop and update individualized plans of care with appropriate visit frequencies
• Coordinate care across interdisciplinary teams and consult with physicians
• Provide patient and caregiver education on disease process, end-of-life care, medications, nutrition, and dietary needs
• Communicate changes in condition to the care team and ensure timely interventions
• Maintain timely clinical records and comply with infection control standards
• Support transitions in levels of care and referrals between home health, palliative, and hospice
