Job summary

Gentiva Hospice is seeking a Hospice RN / RN Case Manager to guide patients and families through every step of their hospice journey, delivering and documenting skilled nursing care in the home or other living settings.

You will perform initial and ongoing assessments, develop comprehensive plans of care, coordinate with physicians, social workers, chaplains, and other team members, and provide compassionate support to patients and loved ones while ensuring accurate documentation and regulatory

Qualifications

• Current RN license in the state of employment.
• CPR certification.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person/telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams with physicians and nurses.
• Educate patients and caregivers on disease processes, self-care, and end-of-life care.
• Communicate changes in condition to the care team for timely interventions.
• Maintain timely clinical records and ensure infection control and compliance.
• Support care continuity by recognizing referrals and transitions between levels of care.
