Job summary

Gentiva Hospice is seeking an experienced Hospice RN / RN Case Manager to lead patient care at home or in alternative settings. You will assess needs, coordinate with the IDG, and deliver skilled nursing per the Plan of Care, guiding families through the hospice journey.

Responsibilities include education of caregivers, documentation, and seamless communication with physicians, social workers, chaplains, and other team members to ensure compassionate, high-quality care.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience; hospice or home health preferred.
• CPR certification required.
• Must have reliable transportation.

Responsibilities

• Serve as primary contact for patients and families in hospice settings.
• Deliver and document skilled nursing care per Plan of Care in home or facility.
• Perform initial and ongoing patient assessments and adjust care.
• Collaborate with IDG to refine care plans.
• Educate caregivers on disease, medications, and end-of-life care.
• Maintain accurate clinical documentation and compliance.
