Job summary

Gentiva Hospice seeks an experienced RN Case Manager to deliver compassionate, patient-centered hospice care in patients' homes and other care settings. You will conduct assessments, implement plans of care, and collaborate with an interdisciplinary team to support patients and families through end-of-life care.

Ideal candidates have RN licensure, at least 1 year of hospice or home health experience, and strong skills in education, symptom management, and communication.

Qualifications

• RN with exposure to hospice or palliative care settings.
• Excellent assessment and documentation skills.
• Ability to educate patients, families, and caregivers.

Responsibilities

• Perform comprehensive patient assessments and reassessments, in person or via telephone, with education for patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Communicate changes in condition to appropriate providers and ensure timely interventions.
