Gentiva is seeking a Hospice RN / RN Case Manager to lead patient-centered care at home, guiding families through the hospice journey with compassion.

You will perform comprehensive assessments, deliver skilled nursing care, coordinate with an interdisciplinary team, educate caregivers, and document outcomes while ensuring quality and compliance across all visits.

Qualifications

• Current RN license in the state of employment.
• CPR certification.
• Valid driver’s license and automobile insurance.
• Minimum 1 year RN experience in hospice, home health, or related fields.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or telephonic settings.
• Deliver and document skilled nursing care aligned with the plan of care.
• Coordinate care with IDG and physicians to refine care plans.
• Educate patients and caregivers on disease progression, medications, and end-of-life care.
• Communicate changes in condition to the care team promptly.
• Maintain timely clinical records and ensure infection control compliance.
• Participate in on-call and support services as required.
