Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home patient care. You will assess patients, deliver skilled nursing, and guide families through the hospice journey with empathy and expertise.

Collaborating with physicians, LPNs, CNAs, social workers, and chaplains, you’ll tailor plans of care, educate caregivers, and maintain precise clinical documentation to ensure high-quality, compliant hospice services.

Qualifications

• Active RN license in the state of employment.
• Minimum 1 year RN experience in acute care, hospice or home health.
• Hospice/palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments with education for families.
• Deliver nursing interventions and symptom management per plan of care.
• Coordinate across IDG to update and refine care plans.
• Document timely records and ensure regulatory compliance.
