Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead in-home nursing care coordination for patients with life-limiting illness. You will perform assessments, implement plans of care, and collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive support.

The role emphasizes education for patients and families, timely documentation, and participation in on-call activities. A current RN license and hospice experience are preferred.

Qualifications

• RN required with active license in the state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in relevant settings (acute, hospice, home health).

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate care with interdisciplinary team.
• Educate patients, families, and caregivers.
• Maintain clinical documentation and infection control standards.
• Participate in on-call and patient support activities.
