Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide skilled nursing care in patients' homes, assisted living, or other environments. You will assess, plan, and implement individualized care with input from an interdisciplinary team to support comfort and quality of life.

You will educate patients and families, coordinate care, and ensure documentation is accurate while adhering to regulatory standards.

Qualifications

• Active RN license in Indiana or able to practice in the state.
• At least 1 year hospice/palliative or home health experience preferred.
• Strong communication and empathy for patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits.
• Deliver skilled nursing interventions and symptom management.
• Develop and update plans of care with visit frequency and risk identification.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers about disease, end-of-life care, and medications.
