Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead comprehensive patient assessments and deliver skilled nursing care in home and community settings. You will coordinate with the interdisciplinary team to refine personalized care plans and support patients and families through education and bereavement guidance.

This full-time role requires RN licensure and hospice experience, with a focus on compassionate, patient-centered care and accurate documentation in the EMR system.

Qualifications

• Current RN license in the state of employment.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency and risk identification.
• Coordinate care with Interdisciplinary Team members (physicians, LPNs, CNAs, social workers, chaplains).
• Educate patients and caregivers on disease processes, end-of-life care, medications and nutrition.
• Document timely clinical records and maintain care plans in the EMR.
