Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to deliver and document skilled nursing care in patients' homes, assisted living, or other living environments. You will lead assessments, coordinate with an interdisciplinary team, and guide families through end-of-life care with empathy and education.

Responsibilities include developing plans of care, documenting updates, and ensuring quality, compliant care.

Qualifications

• Current RN license in the state of employment.
• CPR certification required.
• Driver’s license and auto insurance are required for working in patients’ homes.

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments, including home visits or telephonic evaluations.
• Deliver skilled nursing care per Plan of Care and document care provided.
• Collaborate with the interdisciplinary team to update and refine patient care plans.
• Educate patients and families on disease processes, medications, and end-of-life care.
• Maintain accurate clinical records and ensure infection control and compliance standards.
