Job summary

Gentiva Hospice seeks an RN Case Manager to deliver compassionate, in-home hospice nursing care. You will perform assessments, implement symptom management, and coordinate with the interdisciplinary team to ensure high-quality end-of-life support for patients and families.

Responsibilities include education, care plan development, and timely documentation, with emphasis on empathy and compliance within regulatory standards. Requires active RN license and CPR certification.

Qualifications

• Registered Nurse with active license
• Hospice or palliative care experience preferred
• Excellent patient assessment and communication skills

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Provide skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans and visit schedules.
• Coordinate care with IDG and physicians to ensure seamless transitions.
• Educate patients and families on disease process, medications, and end-of-life care.
