Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in-home hospice care. You will perform comprehensive assessments, implement Plan of Care, and coordinate with physicians, social workers, and other team members to deliver compassionate care.

You will educate patients and families, document progress, and ensure compliance with regulatory standards while supporting transitions in care settings.

Qualifications

• RN license in the state of employment.
• Hospice or palliative care experience preferred.
• At least 1 year of RN experience in acute care, hospice, home health, or similar settings.

Responsibilities

• Perform comprehensive assessments and care planning.
• Deliver skilled nursing interventions per Plan of Care.
• Collaborate with IDG to refine care plans.
• Educate patients and caregivers on disease, medications, and end-of-life care.
• Document clinical records and coordinate visits.
