Job summary

Gentiva Hospice is seeking a dedicated Registered Nurse to lead home-based hospice and palliative care. You will assess patients, provide hands-on nursing care, and develop personalized plans of care in collaboration with an interdisciplinary team.

You’ll educate families, ensure accurate documentation, maintain infection control, and participate in on-call duties to support high-quality end-of-life care.

Qualifications

• RN with an active license in the state of employment.
• CPR certification required.
• Experience in hospice or palliative care preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic reviews.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians and the IDG.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications, nutrition.
• Document timely records and communicate changes in condition to the team.
• Participate in on-call and patient support activities as applicable.
