Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead patient assessments, plan and coordinate end-of-life care in home and community settings. You will work within an interdisciplinary team to ensure compassionate, patient-centered care and timely documentation.

A valid RN license and CPR are required, with at least one year of RN experience in hospice or related fields. You will educate patients and families, manage symptom control, and support caregivers while navigating complex care

Qualifications

• RN license in the state of employment
• CPR certification
• Current automobile insurance and valid driver’s license
• Minimum 1 year RN experience in acute care, oncology, hospice or home health; hospice/palliative care preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments (in-person/telephonic) and educate patients, families and caregivers
• Deliver skilled nursing interventions and symptom management per goals of care
• Develop and update individualized care plans and visit frequency
• Coordinate care across interdisciplinary teams including physicians
• Educate patients and caregivers on disease, self-care, end-of-life care, medications, nutrition and dietary needs
• Communicate changes in condition to the care team
• Support continuity of care across home health, palliative and hospice transitions
• Document timely clinical records including orders, progress notes and care plans
• Maintain relationships with IDG and involve the team as needed
• Participate in on-call and patient support activities
