Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care in home and community settings. You will assess, plan, and deliver hands-on nursing care, coordinating with an interdisciplinary team to honor patient goals and comfort.

You will educate families, document care steps, monitor changes, and support end-of-life needs with empathy and professionalism. A valid RN license and at least 1 year of relevant experience are required; hospice experience preferred.

Qualifications

• Active RN license in Alabama or ability to obtain.
• Minimum 1 year of RN experience in acute care, hospice, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Serve as primary point of contact for patients and families throughout hospice care.
• Deliver hands-on nursing care in home or other care settings per care plans.
• Perform initial and ongoing assessments and adjust plans as needed.
• Collaborate with IDG team to develop and refine care plans.
• Provide education, bereavement guidance, and caregiver training.
• Maintain accurate clinical documentation and comply with infection control standards.
• Support quality improvement and scheduling initiatives.
• Communicate updates to the care team promptly.
