Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead compassionate end-of-life care in patients' homes and local facilities. You will assess, plan, implement, and document personalized care, coordinating with an interdisciplinary team to ensure comfort, dignity, and support for patients and families.

The role emphasizes skilled nursing, education for caregivers, and timely communication of changes in patient condition. A valid RN license, CPR, and hospice experience are preferred.

Qualifications

• Must have current RN license in the state of employment.
• CPR Certification required.
• Current automobile insurance and valid driver’s license.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments; provide education to patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide education on disease process, end-of-life care, medications, nutrition and dietary needs.
