Job summary

Gentiva Hospice in Hot Springs, AR is seeking an experienced Registered Nurse to join as an RN Case Manager. You will be the primary point of contact for patients and families, delivering and documenting skilled nursing care in the home setting and guiding them through the hospice journey.

Collaborate with an interdisciplinary team to develop personalized plans of care, conduct assessments, educate caregivers, and ensure timely, compassionate care.

Qualifications

• RN with active license in the state of employment.
• At least 1 year of RN experience in hospice, home health, or related fields preferred.
• Strong patient assessment, education, and documentation skills.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or field settings.
• Deliver and document skilled nursing care aligned with care plans.
• Develop and update Plan of Care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease process, medications, and end‑of‑life care.
