Job summary

Gentiva Hospice is seeking an experienced Registered Nurse to join our team as an RN Case Manager. You will lead comprehensive home-based hospice care, coordinating with physicians, social workers, chaplains and other disciplines to deliver personalized care aligned with each patient’s Plan of Care.

The role emphasizes clinical excellence, compassionate communication, and timely documentation to support patients and families through life-limiting illness.

Qualifications

• Registered Nurse license for the state of employment.
• Minimum 1 year experience as an RN in acute care, oncology, long-term care, hospice or home health; hospice/palliative care preferred.
• Current CPR certification; valid driver’s license and auto insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments, teaching patients, families and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care incl. visit frequency.
• Coordinate care across interdisciplinary teams including physicians.
• Educate patients and caregivers on disease, self-care, end-of-life care, meds, nutrition.
• Communicate changes in condition to the care team and coordinate timely interventions.
• Maintain accurate clinical records and ensure infection control and compliance.
• Support IDG and participate in on-call and patient support activities.
