Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to provide compassionate, in-home nursing care and to lead patients and families through their hospice journey. You will assess, plan, implement, and document care aligned with goals of care, coordinating with an interdisciplinary team and ensuring comfort and dignity at every step.

Qualifications

• Current RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Ability to work in patients' homes and manage care plans.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or clinic settings.
• Deliver and document skilled nursing care per individualized plans of care.
• Coordinate with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and families on disease, medications, end-of-life care, and self-care.
• Maintain timely clinical records and adhere to infection control standards.
• Participate in on-call and patient support activities as applicable.
