Job summary

Gentiva Hospice is seeking a dedicated Registered Nurse to serve as a Hospice RN / Case Manager. The role focuses on delivering comprehensive, patient-centered nursing care in patients' homes and other care settings, guided by an individualized Plan of Care.

You will collaborate with an interdisciplinary team to coordinate and adjust plans, ensuring compassionate, compliant care and family support throughout the hospice journey.

Qualifications

• Current license to practice nursing in the state of employment.
• CPR Certification.
• Hospice or palliative care experience preferred.
• Minimum of one year RN experience in acute care, hospice, or home health.

Responsibilities

• Be the primary point of contact for patients and families and guide them through the hospice journey.
• Deliver skilled nursing care in home or other settings and document plans of care.
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with physicians, LPNs, CNAs, social workers and chaplains to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Teach caregivers practical and emotional care techniques.
• Communicate updates to the care team and ensure timely interventions.
• Maintain clinical records and follow infection control standards.
