Arkansas Hospice, Inc. is seeking an experienced RN Case Manager in Hot Springs, AR to oversee and participate in clinical interventions for an assigned hospice caseload.

The role emphasizes care coordination, symptom management, and collaboration with an interdisciplinary team. The position requires RN licensure in Arkansas, at least one year of relevant nursing experience, and a valid AR driver’s license.

Qualifications

• Education: Graduation from an accredited school of nursing; BSN preferred.
• Experience: Minimum of one year nursing experience in a hospital, home health, hospice, or related program.
• Licensure: Licensed as a Registered Nurse (RN) in Arkansas.
• Additional Requirements: Valid Arkansas driver’s license, personal automobile, automobile insurance required.

Responsibilities

• Manages patient care after admission and assigns to a designated RN Case Manager.
• Assists in the admission process as requested.
• Coordinates and collaborates with interdisciplinary team for the plan of care.
• Updates the Plan of Care regularly and keeps team informed of patient status.
• Facilitates communication with patients/families and other healthcare providers.
• Maintains contact with attending physician and community organizations regarding resources.
• Monitors pain and symptom control to ensure patient comfort.
• Rotates on-call duties to provide 24-hour service.
• Attends required meetings and participates in quality improvement activities.
