Zusammenfassung

Health Connect America is hiring a Family Development Specialist to support clients and families in developing and implementing treatment plans within homes and communities. You will provide therapeutic interventions and coordinate services to meet each client’s needs.

The role emphasizes collaboration with a treatment team, strong documentation, and active outreach to recruit referrals while ensuring regulatory compliance. This is a full-time, community-based position.

Qualifikationen

• Bachelor's degree in a human services discipline is required.
• Minimum of two years experience working with children and/or families (internships/volunteer work acceptable).

Aufgaben

• Develop individualized treatment plans and coordinate care with the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions and skill-building.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families to services and resources per treatment plan.
• Serve as liaison with professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of client interactions and progress notes per policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training and professional development for quality service delivery.

Ausbildung

Bachelor's degree in a human services discipline (Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field)
