Job summary

Health Connect America, Inc. is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to services in home and community settings.

You will maintain documentation, participate in weekly meetings, recruit referrals, ensure regulatory compliance, and collaborate with a treatment team to deliver evidence-informed care.

Qualifications

• Bachelor's degree in a human services discipline such as Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field is required.
• Minimum of two years experience working with children and/or families.

Responsibilities

• Develop individualized treatment plans and coordinate care for clients and families with input from the treatment team and Clinical Supervisor.
• Provide individual, group, and family therapeutic interventions, and skill-building as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families with specific services and resources identified in the treatment plan.
• Serve as a liaison with other professionals, agencies, and community resources to meet client needs.
• Maintain accurate documentation of all client interactions and progress notes in compliance with policy and regulatory standards.
• Actively recruit new referrals to maintain a full caseload.
• Provide 24/7 on-call support as required by the program.
• Ensure compliance with state regulatory bodies and COA standards.
• Participate in ongoing training to stay current with best practices.
