Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by developing and implementing treatment plans, providing therapeutic interventions, and linking them to necessary services in home and community settings.

The role requires effective communication, thorough documentation, and active recruitment of new referrals, with compliance to COA standards and state regulations; a bachelor’s degree in a human services field and at least two years’ experience

Qualifications

• Bachelor’s degree in a human services discipline is required.
• Minimum of two years of experience working with children and/or families. Internship and volunteer work may count toward this requirement.
• Experience working with families in home or community settings is preferred.

Responsibilities

• Develop individualized treatment plans and coordinate care with the treatment team.
• Provide individual, group, and family therapy interventions as outlined in the Treatment Plan.
• Attend and participate in weekly Treatment Team meetings.
• Link clients and families to services and resources identified in the plan.
• Maintain accurate documentation of client interactions and progress notes in compliance with policy and regulatory standards.
