Job summary

Health Connect America is seeking a Family Development Specialist to support clients and families by designing and implementing treatment plans and delivering therapeutic interventions in home, community, or office settings. The role emphasizes collaboration with treatment teams, regulatory compliance, and active referral recruitment.

The position requires a bachelor’s degree in a human services field and at least two years of experience with children or families.

Qualifications

• Bachelor’s degree in a human services field is required.
• Minimum of two years experience with children and/or families.

Responsibilities

• Develop and implement treatment plans for clients and families.
• Provide individual, group, and family interventions and skill-building.
• Attend weekly Treatment Team meetings.
• Link clients to services and resources as identified in plans.
• Act as liaison with professionals and community resources.
• Maintain accurate, timely documentation per policy.
• Recruit new referrals to maintain caseload.
• Provide 24/7 on-call support as required.
• Ensure compliance with state regulations and COA standards.
• Participate in ongoing training and professional development.
